The Big-RIVERS (Regional Integrated Vision for Effective Renal Support) Project aims to implement and evaluate a new regional, digitally-enabled model of multidisciplinary care to facilitate effective care coordination and improve patient journeys and health outcomes for First Nations peoples in the Big Rivers Region. Directly responding to the needs of Aboriginal Community Controlled Health Services (ACCHS), this project centres First Nations peoples, remote and transient patients, and people at risk of chronic conditions. We will develop efficient workflows for active monitoring, identification, management of chronic conditions by utilising Territory Kidney Care (TKC), a clinical decision support tool that integrates data from primary and tertiary services. Working with an established consortium of partners and building on TKC as a digital platform, we focus on the Big Rivers Region with a view to generate sustainable models of care that can be scaled to other regions in the NT and beyond.
